1 Week Before Surgery
Start here. These are the things I tell every single patient at Karachi Piles Clinic at their pre-surgical consultation.
- Stop blood thinners. If you are on aspirin, clopidogrel, warfarin, or even fish oil supplements — stop them at least 7 days before. Always confirm this with me first.
- Stop smoking. I know this one is hard. But smoking reduces blood flow and slows healing. Even stopping for one week makes a difference.
- Blood tests. We usually need CBC, PT/APTT, and blood sugar. If you are diabetic, your sugar should be controlled before we proceed.
- Tell me everything you are taking. Herbal medicines, vitamins, everything. Some herbal supplements increase bleeding risk.
The Night Before Surgery
This is where most patients make mistakes. Especially with eating and drinking.
For spinal or general anaesthesia — which we use for flap procedures — you must be completely empty stomach (NPO) for at least 6 hours before your surgery time. No food. No water. Not even a sip.
For local anaesthesia procedures like Laser Sinus Closure, you can usually have a light meal 2 hours before. But confirm this with me at your pre-op visit because it depends on your specific procedure.
Also — shave the area the night before, or at least the morning of surgery. A clean operative field reduces infection risk significantly. I have noticed that patients who do this carefully have fewer post-operative wound issues.
Morning of Surgery — Checklist
On the day itself, here is what to do:
- Shower with regular soap — do not apply any cream or lotion to the area
- Wear loose, comfortable clothing. You will not want tight jeans after the procedure, trust me
- Bring a responsible adult with you. You cannot drive yourself home after any surgery
- Bring your previous reports, blood tests, and any prescription medicines
- Leave jewelry and valuables at home

Avoid heavy, oily food the day before surgery. A light dinner — daal, roti, some yoghurt — is ideal.
What NOT to Do
Also — and I cannot stress this enough — do not Google your surgery the night before. I have seen patients come in with completely wrong information from random websites and they were terrified for no reason. Trust your surgeon. Trust the process.
