Insurance Coverage for Laser Surgery in Karachi — How It Works
Karachi Piles Clinic — led by Dr. Abdullah Iqbal, Pakistan’s first laser proctologist — accepts all major Pakistani insurance panels including EFU Life, Jubilee Insurance, Adamjee, IGI, State Life, Askari General, TPL Life, Habib Insurance and most corporate health plans. Laser surgery for piles (bawaseer), fissure, fistula (bhagandar), pilonidal sinus, and perianal abscess is covered under your policy’s surgical benefit — but pre-authorization is mandatory before the procedure.
The process is structured and patient-friendly. You first come in for a consultation where Dr. Abdullah Iqbal examines you, confirms the diagnosis, and — if surgery is genuinely needed — writes a formal surgical prescription. You then WhatsApp three documents to 0333-2877351: the prescription, your CNIC, and your insurance card. Our clinic team verifies your eligibility directly with your insurance provider and confirms your approval status — usually within 24 to 48 hours. There is no paperwork chase for you, no phone calls to your HR or insurance company, and no surprise bills on surgery day.

Why Pre-Authorization Is Required Before Surgery
If you have never used your health insurance for a surgical procedure before, the term “pre-authorization” can sound intimidating. It is not. Pre-authorization is simply a formal written confirmation from your insurance company that says: “Yes, we will cover this surgery, for this patient, at this clinic.” Without it, even if your premiums are fully paid and your policy is active, the insurance company can refuse to settle the bill afterwards — leaving you with a financial surprise no patient deserves.
Every health insurance policy in Pakistan that includes hospitalization or surgical benefits requires pre-authorization for planned (non-emergency) procedures. This is not unique to Karachi Piles Clinic — it is how the entire industry works. Insurance companies need to verify three things before they agree to pay for a procedure:
- Is the procedure medically necessary? A doctor’s written prescription is required as proof. The insurance company needs a written clinical assessment from a qualified surgeon stating that surgery is the appropriate treatment for the diagnosed condition.
- Is the procedure covered under your specific policy? Policies vary widely. Some cover all surgical procedures without sub-limits, others have category sub-limits, and a few exclude specific procedures entirely. Pre-authorization tells you exactly what your policy will pay and what (if anything) you will need to settle yourself.
- Is the patient eligible at this point in time? The insurance company confirms the policy is active, premiums are paid up to date, waiting periods are satisfied, and no exclusions apply to your case.
The good news: at Karachi Piles Clinic, the pre-authorization process is designed to be as simple as possible. You do not need to call your insurance company yourself, fill out lengthy forms, or chase paperwork. Our clinic team handles the entire verification process on your behalf — you just send three documents via WhatsApp, and we do the rest.
The Complete Pre-Authorization Process — Step by Step
Here is exactly what happens, from your first contact to the day of your surgery, when you use insurance for treatment at Karachi Piles Clinic:
Book a Consultation with Dr. Abdullah Iqbal
The process always begins with an in-person consultation. Book by calling 0333-2877351 or filling the form at the top of this page. Consultation days are Tuesday, Thursday, and Saturday from 5:00 PM to 8:00 PM at Karachi Piles Clinic, Shahid-e-Millat Road, Karachi. Consultation fee is PKR 2,000 — paid at the clinic. (Consultation fees are excluded under most insurance policies, so plan to pay this directly.)
Clinical Examination and Diagnosis
During your consultation, Dr. Abdullah Iqbal conducts a thorough clinical examination to confirm the diagnosis. For proctology conditions, this may include a digital rectal examination and, where indicated, a proctoscopy. The examination typically takes 20 to 30 minutes. Dr. Abdullah explains the findings clearly, discusses all treatment options, and answers any questions you have. There is no pressure to commit to surgery — many patients are managed with medication, dietary changes, or in-clinic procedures.
Surgical Prescription (Only If Surgery Is Needed)
If after examination Dr. Abdullah determines that laser surgery is the right treatment for your condition, he writes a formal surgical prescription. This is a clinical document containing your diagnosis, the recommended procedure, the medical justification, and the surgeon’s signature. This prescription is the foundation of your pre-authorization application — without it, no insurance company will approve surgical coverage. If surgery is not needed, you will receive a prescription for non-surgical treatment instead, and the pre-authorization step does not apply.
Send Three Documents via WhatsApp
Once you have the surgical prescription, the pre-authorization process begins. Send three documents to our WhatsApp at 0333-2877351:
- Surgical prescription issued by Dr. Abdullah Iqbal (photo with your phone)
- CNIC — clear photos of both the front and back side
- Insurance / policy card — clear photos of both sides if it has information on the back
You can send these at any time, day or night. Our team will receive them and start the verification within working hours.
Eligibility Verification and Approval Confirmation
Our clinic team takes your submitted documents and contacts your insurance company directly. We submit the surgical prescription, confirm your policy is active, check the coverage limits for your specific plan, and request formal pre-authorization for the procedure. This process typically takes 24 to 48 hours — some panels respond same-day, while a few may take up to 3 working days. You will then receive your status via WhatsApp:
- Fully approved — your procedure is covered, you can schedule surgery
- Partially approved — the insurance covers a portion of the cost; we explain exactly what is covered and what you need to pay
- Declined — we explain the reason and discuss alternatives (see section below)
Surgery Day — Bring Originals
Once approval is confirmed, you can schedule your surgery. Procedures are typically scheduled within 1 to 2 weeks of approval, depending on your preferred date. On the day of surgery, bring originals of all documents: CNIC, insurance card, prescription, and the pre-authorization approval letter we provide you. For corporate panels, also bring the HR letter or employment verification.

What Happens If Pre-Authorization Is Declined?
A declined pre-authorization is not the end of the road — there are almost always options. Here are the most common reasons for denial and what you can do about each:
1. The policy has a waiting period for surgical procedures. Many new insurance policies (especially those purchased within the last 6 to 24 months) have a “waiting period” during which surgical benefits are not yet active. If this is the case, you have three choices: wait until the waiting period expires and schedule surgery for that date, pay out of pocket now and claim reimbursement later if your policy allows, or proceed as a self-paying patient with our transparent self-pay pricing.
2. The condition is classified as pre-existing. If you had documented symptoms of your condition before your insurance policy began, the insurance company may classify it as a “pre-existing condition” and exclude it from coverage. You can appeal this by providing medical records showing the condition only became symptomatic after the policy began, or proceed as a self-paying patient.
3. The specific procedure is not covered by your plan. Some economy plans exclude certain categories of surgery. Where possible we recommend an alternative non-surgical management option that IS covered, or you can proceed as a self-paying patient.
4. Documentation issues. Sometimes pre-authorization is declined simply because a document was missing, blurry, or unclear. We will help you correct this and resubmit — this is usually a fast fix.
If pre-authorization is fully declined and there is no workaround, we will discuss the self-pay options transparently. Self-pay pricing for laser procedures at Karachi Piles Clinic is significantly lower than other private hospitals in Karachi, and we offer easy installment options on bank debit and credit cards. The conversation about money is always open and pressure-free — many patients choose to proceed even without insurance because the laser approach offers same-day discharge, less pain, and faster recovery than conventional surgery.
Timeline & Cashless vs Reimbursement
For most patients, the timeline from first contact to surgery date is 2 to 3 weeks:
- Day 1: Call or WhatsApp to book your consultation
- Within 1 week: In-person consultation, examination, and prescription (if needed)
- Same day or day after: WhatsApp documents for pre-authorization
- 24–48 hours later: Approval status confirmed
- 1–2 weeks after approval: Surgery scheduled and performed
Urgent cases — severe pain, bleeding, infection — can be expedited. Dr. Abdullah Iqbal sees walk-in emergency patients during clinic hours, and pre-authorization for urgent cases is typically processed within hours rather than days.
Cashless vs Reimbursement — What’s the Difference?
There are two payment models in Pakistan’s health insurance system, and it helps to understand which one applies to your policy before surgery day:
Cashless treatment means Karachi Piles Clinic bills your insurance company directly. You do not pay anything out of pocket — except for items not covered, like consultation fees, certain medications, or hospital extras. At Karachi Piles Clinic, most major panels offer cashless treatment, including all eight major insurers listed above. This is the most convenient option for patients.
Reimbursement treatment means you pay the clinic upfront, then submit your bills, prescription, discharge summary, and other documents to your insurance company. The insurer reimburses you within 4 to 12 weeks. Reimbursement is used when (a) your specific policy does not have cashless arrangements, (b) the procedure is partially covered and you have agreed to settle the difference, or (c) certain corporate plans require this workflow by design.
When you contact us for pre-authorization, we confirm whether your specific policy operates on cashless or reimbursement basis — so there are no surprises on surgery day.
