Let me be honest with you. In my years of practice at Karachi Piles Clinic, one of the most common things I hear from patients is — “Doctor, mujhe nahi pata tha ke meri bawaseer itni aage ja chuki hai.” And that is the real problem. People ignore it. They hope it goes away. It does not always go away.
So today, I want to explain piles grades — from Grade 1 all the way to Grade 4. Because knowing your grade is the first step toward the right treatment. Also, this will help you understand when you actually need surgery and when you might manage without it.
What Are Piles Grades, Exactly?
Piles — jo ke hemorrhoids bhi kehlaati hain — anal area ki swollen veins hoti hain. Yeh internal ya external ho sakti hain. The grading system I use is based on how far they come out and whether they go back in on their own. Simple as that.
I have seen Grade 1 patients who were fine with just dietary changes. And I have seen Grade 4 patients who waited too long and needed emergency treatment. The difference? Awareness.
Grade 1 Piles — Shuruat Ka Stage
In Grade 1, the veins inside the anal canal are swollen but they do not come outside. You cannot see or feel them from the outside. Most patients at this stage have bleeding — small amounts of bright red blood on tissue paper or in the toilet. No pain, usually.
Kya aapke saath bhi aisa hua hai? Khoon dekh ke dar gaye, phir theek ho gaya — aur bhool gaye? Yahi problem hai.
In my experience, Grade 1 responds very well to dietary changes. High fiber, more water, no straining. Sometimes I prescribe Daflon tablets for a few weeks. That is often enough. But — and this is important — it does not mean you should ignore it. Grade 1 can become Grade 2, then 3, then 4 if you keep delaying.
Grade 2 Piles — Theek Hai, Lekin Dhyan Do
Grade 2 is when the piles come out during bowel movement but go back in on their own afterwards. You might feel something prolapsing and then disappearing. Sometimes there is itching, discomfort, and yes, bleeding.
At this stage, diet alone is usually not enough. I often recommend band ligation or sclerotherapy along with medications. Some Grade 2 patients also do well with laser treatment — it is quick, same-day procedure, very minimal discomfort.
Grade 3 Piles — Ab Seriousness Hai
Here is where things get serious. In Grade 3, the piles come out during bowel movement and do NOT go back in on their own — you have to push them back manually. This is called manual reduction. If you are doing this regularly, you have Grade 3 piles.
At this grade, I almost always recommend laser treatment or surgical intervention. Medicines and diet will not fix Grade 3. In my opinion, the sooner you get it treated, the better the outcome. I have treated hundreds of Grade 3 patients with laser hemorrhoidoplasty at Karachi Piles Clinic — most go home the same day, back to work in 2 days.

Grade 4 Piles — Yeh Emergency Hai
Grade 4 is the most advanced stage. The piles are permanently outside and cannot be pushed back in. They are often thrombosed — meaning blood has clotted inside — which causes severe pain. I have seen patients come in barely able to sit or walk.
Surgery at this stage is almost always necessary. I have performed laser procedures on Grade 4 patients and results are good, but the recovery is longer than early stages. Also sometimes traditional hemorrhoidectomy is needed depending on the size and complexity.
The real tragedy is that most Grade 4 cases I see could have been treated at Grade 2 with a simple procedure. Delay costs you more — in pain, in money, in time off work.
How to Know Which Grade You Have
Honestly, you cannot fully determine your grade yourself. Clinical examination — and sometimes proctoscopy — is needed. But here is a rough guide:
- Blood on tissue, no swelling felt outside = likely Grade 1
- Something comes out during bowel movement, goes back on its own = likely Grade 2
- Needs manual pushing back in = Grade 3
- Cannot be pushed back, severe pain = Grade 4

Treatment Based on Grade — My Recommendation
Based on my experience as Pakistan first laser proctologist, here is what I generally recommend:
Grade 1 and early Grade 2: start with dietary changes — high fiber, 8 glasses of water, no straining. Add Daflon or similar medication. Follow up in 4 weeks. Most patients improve significantly.
Late Grade 2 and Grade 3: proceed to laser treatment. Band ligation is also an option for Grade 2. Results are excellent at this stage with minimal downtime.
Grade 4: surgical treatment is almost always required. We discuss the best approach case by case at our clinic.

Yeh vary karta hai. Kuch patients mein saalon mein progress hoti hai, kuch mein maheenon mein — especially if diet is poor, constipation is chronic, or there is pregnancy. Early treatment prevents progression.

I have been treating piles for years now. And the one thing that frustrates me most is seeing Grade 4 patients who say “main socha ghar pe theek ho jayega.” It rarely does, at that stage.
