Most of my patients, when they first come to me at Karachi Piles Clinic, they have already tried every totkay, every YouTube remedy. But no one told them the real issue. The food. Honestly, diet is the number one reason why piles and fissures keep getting worse — and also the first thing I fix before anything else.

So let’s talk about it properly. No long lecture. Just what actually works. And this applies to both — bawaseer (piles) and anal fissure. Because the dietary advice for both overlaps quite a lot.

In my experience, most Karachi patients eat maybe 8 to 10 grams of fiber daily. Your gut actually needs 25 to 30 grams. That gap — that difference — is exactly where the problem lives. And fissure patients suffer even more because a hard stool physically tears the anal lining every single time. So every meal either helps healing or delays it.

1. Spicy Food — Mirchi, Achaar, Chatni

Capsaicin in chillies passes through your gut without fully digesting. When it exits — it burns inflamed tissue directly. For fissure patients especially, this is very painful. Switch to zeera, haldi, dhania. Mild. Anti-inflammatory. Actually better for flavor too, in my opinion.

2. Maida — Naan, White Bread, Biscuits

Zero fiber. I see this every single day in Karachi patients — naan for lunch, naan for dinner. Refined flour slows your gut completely. Switch to wheat roti. Just this one change can make a real difference within a week.

3. Fried Snacks — Samosas, Pakoras, Chips

High fat, zero fiber. Slows digestion. The longer food sits in your intestine, the more water gets absorbed from stool — and harder it becomes. That is what causes the straining that worsens both piles and fissures.

4. Too Much Chai and Coffee

Caffeine is a diuretic — it pulls water out of your body, including your colon. And fiber without water doesn’t work at all. Aur chai tou sab peetay hain — I understand. But try replacing even one cup with warm lemon water in the morning. That small change, I have seen it help many patients.

5. Mithai, Cold Drinks, Ice Cream

Sugar slows gut motility. These are also completely fiber-free. Replace with soaked dates (khajoor) or fresh fruit. Naturally sweet. And actually helpful for your gut.

6. Red Meat Without Sabzi or Daal

I am not saying stop eating meat. I am saying — balance it. Red meat has no fiber. Every meat dish must have daal or vegetables on the side. That combination is what protects you.

Avoid karna alone enough nahi hai. You have to add fiber actively. Here is what I tell my patients at Karachi Piles Clinic:

Fissure patients need to be even more careful than piles patients. Why? Because every hard stool passage reopens the tear. It’s like a wound that keeps getting scratched. So for fissure, I add a few extra points on top of the general advice above.

First — water intake is non-negotiable. Minimum 2.5 liters daily. Not negotiable. Second — isabgol (psyllium husk) at night in warm water is very helpful. It softens stool without stimulating the gut. Third — avoid anything constipating: unripe bananas, rice in large quantity, boiled eggs alone without vegetables. And fourth — sitz bath twice daily with warm water helps the fissure heal faster and reduces spasm. Diet plus sitz bath together work better than diet alone.
