Bawaseer Ka Ilaj Choose Karte Waqt Sabse Pehle Kya Dekhna Chahiye?
In my experience, the single biggest mistake patients make is starting treatment before knowing their Grade. Aap Grade 1 ke liye laser karwa lo — it works. Aap Grade 4 ke liye sirf cream lagao — it won’t. So the treatment must match the stage. Below are the 9 options we use — from simplest to most complex.
Bawaseer Ke Ilaj Ke 9 Tareeqay Kya Hain — Aur Kaun Sa Kab Kaam Karta Hai?
1. Dawai aur Cream (Conservative Treatment)
Grade 1 ka pehla option. Daflon, Hedesa cream, stool softeners — these reduce swelling and bleeding temporarily. But dawai bawaseer ko khatam nahi karti. It just controls symptoms. Agar 4 se 6 hafte mein improvement nahi — age badhna zaroori hai. See my article on Daflon tablets dosage for details.
2. Sclerotherapy (Injection Treatment)
A chemical is injected into the hemorrhoid which shrinks it. Simple, fast — 5 to 10 minutes in OPD. Grade 1 to 2 ke liye. But results are not always permanent and multiple sessions sometimes needed. I use this occasionally for selected early cases.
3. Infrared Coagulation (IRC)
Infrared light is used to burn the base of the hemorrhoid. Quick, painless, no surgery. Grade 1 to 2 only. Not very commonly done in Pakistan but available. Good for elderly patients who cannot tolerate bigger procedures.
4. Radiofrequency Ablation
Heat energy destroys the hemorrhoid tissue. Similar to IRC but uses radiofrequency instead of light. Painless, no incision. Works best for small internal piles. Recovery is fast — same day discharge.
5. Band Ligation
A small rubber band is placed around the base of the hemorrhoid. Blood supply cuts off — piles shrinks and falls off in 7 to 10 days. Very effective for Grade 1 to 3 internal piles. One of my most used options in clinic. Read more about band ligation treatment in Karachi.
6. Laser Treatment
Laser energy shrinks and seals the hemorrhoid from inside. No big cut. Minimal bleeding. 20 to 30 minutes. Grade 2 to 3 ke liye excellent. Recovery 3 to 5 days. In my opinion, yeh Grade 3 ke liye best option hai in most cases. Detailed guide: laser treatment for piles in Karachi.
7. Doppler-Guided HAL (Hemorrhoidal Artery Ligation)
Ultrasound finds the exact artery feeding the hemorrhoid — then it is tied off. No cuts, very low pain. Good for internal Grade 2 to 3 piles. Not effective for external hemorrhoids. Not available everywhere in Pakistan but we do offer it.
8. Stapler Surgery (MIPH)
A circular stapler repositions prolapsed internal hemorrhoids. Less pain than open surgery, faster recovery. But in some cases piles can return, and rarely anal stenosis can occur. I recommend this mainly for Grade 3 to 4 cases with significant prolapse.
9. Open Hemorrhoidectomy (Traditional Surgery)
The most complete option. Piles are surgically removed. Grade 4, large external hemorrhoids, or recurrent cases — yahan surgery zaroori hoti hai. Recovery is 2 to 4 weeks and there is more post-op pain. But it is a permanent solution when done properly. We now use LigaSure which makes it significantly less painful than old methods.
