But there’s more to it than just laser. Over the last few years, the treatment of hemorrhoids has changed quite a lot. In my opinion, Pakistani patients — especially in Karachi — now have access to the same advanced options that were only available abroad just a decade ago.

The old way — open hemorrhoidectomy — still has its place, but it was the only option for decades. Painful recovery. Two to four weeks off work. Significant post-operative discomfort. For many patients, especially in Karachi where people simply can’t afford long absences from work or family, this was a real problem.

I have noticed that many patients used to delay treatment specifically because they were afraid of surgery. They’d come in with Grade 3 or even Grade 4 hemorrhoids — something that could have been treated much more easily at Grade 2 — simply because fear stopped them. That delay costs them more pain, more money, and longer recovery. This is exactly why newer treatment options matter so much.

Laser Treatment for Piles Kya Hoti Hai Aur Yeh Purani Surgery Se Kaise Alag Hai?

Laser treatment is the most significant advancement in piles treatment in recent years. In simple terms — a thin laser fiber is guided directly into the hemorrhoid tissue. The laser energy shrinks the blood supply of the hemorrhoid from the inside, causing it to gradually collapse and be reabsorbed by the body. No big cut. No open wound.

Compare this to traditional surgery where the hemorrhoid is physically cut out and the wound either stitched or left open to heal — which takes weeks and hurts quite a bit. Laser is fundamentally different. The procedure takes about 20 to 30 minutes. Most patients go home the same day. And in my experience at Karachi Piles Clinic, the majority of patients are back to normal activity within 3 to 5 days.

DGHAL (Doppler-Guided Hemorrhoidal Artery Ligation) Kya Hai Aur Kya Yeh Pakistan Mein Available Hai?

DGHAL is one of the newer minimally invasive options. In this procedure, a small Doppler ultrasound probe is inserted to locate the exact arteries feeding the hemorrhoid. Those arteries are then tied off (ligated) — which cuts the blood supply and causes the hemorrhoid to shrink naturally over time.

The key advantage is precision. Instead of removing tissue, you are just stopping the blood supply. Recovery is quite comfortable and the procedure has good long-term results for Grade 2 and 3 hemorrhoids. In my opinion, DGHAL combined with mucopexy (stitching the prolapsed tissue back up) is excellent for patients who want to avoid any cutting entirely.

Band Ligation Aur Sclerotherapy — Kya Yeh Abhi Bhi Relevant Hain?

Yes, absolutely. These are not outdated options. Band ligation in particular remains one of the most reliable clinic-based treatments for Grade 1 and 2 hemorrhoids. A small rubber band is placed at the base of the hemorrhoid, cutting off its blood supply. The hemorrhoid dries up and falls off within about a week — with very little pain for most patients.

Sclerotherapy — where a chemical solution is injected into the hemorrhoid — is similarly effective for small, early-stage cases. It’s quick, 5 to 10 minutes, and the patient goes home same day. However, it’s generally not my first choice for larger hemorrhoids because the results are less predictable at that stage.

Meri practice mein, mein har mareez ka Grade aur symptoms dekh kar ilaj decide karta hun. Koi aik nuskhah sab ke liye theek nahi hota. Agar aap Grade 1 ya 2 hain — band ligation ya laser dono behtar options hain. Grade 3 ke liye laser meri pehli pasand hai. Home management has a real role — but only for very early, Grade 1 cases, and only as a first step. Sitz bath (warm water soaking twice daily), a high-fiber diet, stool softeners, and reducing straining can significantly help with early bleeding and discomfort. But here is the honest answer — if your symptoms have been there for more than 2 weeks, if bleeding is frequent, or if you feel a lump or prolapse, home care is not going to be enough. You need a proper examination. Delaying is what I see cause the most damage in my patients. A Grade 2 that could have been treated easily in the clinic becomes Grade 3 or 4 because someone waited a year on home remedies.

This is something I talk about with every single patient after their procedure. Piles can come back — not because the treatment failed, but because the underlying habits that caused them in the first place are still there. Constipation. Hard stools. Straining on the toilet. Sitting for long hours. Low fiber diet.
